Clinical Observation on the Upper Gastrointestinal Tract Hemorrhage

Abstract
A total of 92 cases of the upper gastrointestinal bleeding were clinically reviewed, who were admit
ted to the Department of Surgery, Chonnam University Hospital during 3 years from Jannuary 1968 to
December 1970.
The results were as follows,
1) The predominant diseases producing upper gastrointestinal hemorrhage were Gastric ulcer(32.5%),
Duodenal ulcer(21.7%), Stomach ca(16.2%), Gastritis(11.1%), and Esophageal varices(9.8%),
in this order.
2) The greatest incidence of the disease occurred in the age group of fifth decade which comprised
38%, and as 3.5 times greater prevalence in male as in female.
3) The analysis of mode of upper gastrointestinal hemorrhage revealed 42 cases (46%) in hemate
mesis, 20 cases (22%) in melena, and 30 cases (32%) in hematemesis and melena.
4) Among the 92 cases, 66 cases had massive bleeding and a mortality rate of 21%, 19 cases had
moderate bleeding and a mortality rate of 10%, and 7 cases had mild bleeding without any mortality.
5) Among the 92 cases, operation was done in 64 cases and conservative treatment was done in 28
cases. The mortality rate for patients receiving surgical treatment was 11%, which the conservatives,
mortality rate for those treatment was 32%.
6) Death occurres in 16 patients, with a mortality rate of 17%, of which 9(32%) were conservative
group and 7(11%) operative group.
7) The cause of death were mainly due to pulmonary complication, shock and hepatic coma.
